Patient Participation Group

Our Patient Participation Group, Crystal Peaks PPG, is encouraging patients to give their views about how the practice is doing. We hold ad-hoc face-to-face meetings with patients about changes to the practice and any items the patients feel they would like to discuss.

Are you interested in leaving your email contact details? If so, please fill in the form attached and drop off at reception. Your email will be used to invite you to come in to any upcoming meeting. These meetings are held on alternate afternoon and evenings to accommodate all of our patients. Alternatively you can collect a form from reception.

If you do not have access to email, please contact reception who will add your name to our PPG list and we will contact via telephone.
